Areas of Service

Services are individualized and may include support and skills training in the areas of wellness, exercise, nutrition, health care coordination, medication management, self-care, meal preparation, housekeeping and household tasks, shopping, budgeting, constructive use of leisure time, public transportation, strategies for obtaining educational and vocational opportunities, engagement in social and recreational activities, relationship skills and self-advocacy.

 

Job skill development

Our primary aim is fostering self-esteem and community inclusion through personalized employment, education, and training programs. Our focus is on empowering individuals to enhance their existing skills, rather than simply job placement. We achieve this through tailored career guidance and a range of skill development opportunities.


Regular Evaluations

Conduct regular assessments of the individual's progress to determine if they are meeting the established milestones. If the individual has achieved their goals or no longer requires intensive individual services, they may be encouraged to transition to less restrictive settings.



Family and Community Involvement

Involve family members and support networks throughout the rehabilitation journey. Engaging them in the process can help build a strong foundation for continued support post-program.



Resource Optimization

Maximize the utilization of available resources and services to address the individual's needs efficiently. Streamline service delivery and explore partnerships with external organizations to ensure timely access to necessary resources.




Goal-Oriented Planning

Emphasize setting clear and measurable goals with specific time frames for achievement. Periodically review progress towards these goals and adjust the rehabilitation plan accordingly.


Transition Planning

From the outset, including transition planning as a core component of the rehabilitation process. Prepare individuals for the eventual move back into the community by identifying and establishing appropriate community-based supports.
